Triple-negative breast cancer (TNBC) remains a highly aggressive subtype with limited targeted treatment options. Oncology nurses care for patients who often face significant physical, emotional, and psychosocial challenges, including treatment-related side effects and psychological distress. In addition to pharmacologic management of these symptoms, nurses and patients frequently turn to integrative approaches to enhance quality of life and support patients throughout treatment. However, the benefit of these integrative approaches is not always well understood.
In a recent review, Dr. Shlomit Ein-Gal and colleagues examined the role of integrative approaches in supporting patients with TNBC. Given the aggressive nature of TNBC and the physical and psychological challenges associated with its treatment, integrative strategies are increasingly being explored to enhance quality of life and potentially improve clinical outcomes.
Intermittent Fasting and Fasting-Mimicking Diets
Dietary interventions, particularly intermittent fasting (IF) and fasting-mimicking diets (FMDs), have shown promise in TNBC. Preclinical studies suggest that IF can reduce tumor cell proliferation, migration, and invasion, while also decreasing pro-inflammatory signaling. Clinically, short-term fasting around chemotherapy cycles has been associated with reduced fatigue and improved treatment tolerance. For example, a pilot crossover study demonstrated that a 60-hour fasting regimen, beginning 36 hours before and ending 24 hours after chemotherapy, significantly improved fatigue scores compared with non-fasted cycles. Larger cohort studies also suggest that prolonged nightly fasting of more than 13 hours may reduce recurrence risk. In patients with advanced TNBC, combining FMD with chemotherapy has shown encouraging survival benefits.
Nutritional Supplements
Several dietary supplements have been investigated for their potential role in TNBC management. Vitamin D has been linked to improved chemotherapy response and a lower risk of TNBC, particularly in premenopausal women. Omega-3 fatty acids, specifically eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), may inhibit tumor growth and metastasis while alleviating treatment-related side effects such as neuropathy and cognitive dysfunction. Maitake D-fraction, derived from the Maitake mushroom, has demonstrated antitumor activity and immune-modulating effects in preclinical models, with findings from clinical studies supporting safety and immune benefits. Early studies also suggest that cannabidiol (CBD) may enhance chemotherapy efficacy through modulation of immune response pathways. Selenium has shown cytotoxic effects on cancer cells and inhibition of metastasis in preclinical research. Withania somnifera, commonly known as ashwagandha, contains withaferin A, which may promote apoptosis, suppress tumor growth, reduce metastasis, and enhance chemotherapy effectiveness while also supporting stress reduction.
Specific Foods and Dietary Patterns
Certain foods may have protective effects for patients with TNBC. Long-term consumption of soy, green tea, seafood, and berries has been associated with reduced tumor burden and metastasis in preclinical models. Conversely, consumption of alcohol, high-fructose diets, and highly processed foods may increase TNBC risk. Diets emphasizing fruits, vegetables, whole grains, fish, legumes, and unsaturated fats, such as the Mediterranean, DASH, and an American Cancer Society–recommended diets, have been linked to improved survival and lower recurrence rates.
Mind-Body Medicine and Psychosocial Support
Mind-body interventions, including meditation, yoga, and guided imagery, have been shown to alleviate anxiety, depression, and overall stress while improving quality of life. Structured integrative programs that combine mindfulness, yoga, exercise, nutrition counseling, and cognitive behavioral strategies can support both mental and physical resilience during chemotherapy, helping patients cope with the emotional and physical demands of treatment.
Lifestyle Interventions
Regular physical activity is associated with reduced TNBC risk, lower recurrence rates, and improved survival. Exercise may influence cancer biology by lowering growth-promoting factors such as insulin-like growth factor 1 and enhancing immune function. Preclinical studies have also shown that serum from patients participating in structured exercise programs can inhibit TNBC cell proliferation, highlighting potential biological benefits beyond general fitness.
Complementary Modalities
Additional integrative strategies include traditional Chinese medicine (TCM), acupuncture, and clinical hypnosis. TCM herbal formulas may reduce recurrence and metastasis, although individualized treatment makes standardization challenging. Acupuncture has demonstrated benefits in terms of pain, fatigue, hot flashes, sleep, and anxiety, supporting overall physical and emotional well-being. Clinical hypnosis has been shown to alleviate chemotherapy-related side effects and improve quality of life.
Nursing Considerations
Oncology nurses play a critical role in integrating these complementary strategies into TNBC care. Nurses can provide education about evidence-based interventions, assess patient interest and readiness for integrative approaches, monitor for safety and interactions with standard therapies, and facilitate access to an integrative medicine team.
